Information About Your Family History


Does anyone in your family have a medical condition? It’s important to inform your health care provider about any health conditions that occurred in your family or close relatives. This is because family history may influence the development of certain health conditions, such as stroke, diabetes, or even cancer. Your doctor may assess risk factors based on your family’s medical history and provide recommendations to prevent the development of these conditions.

List of Health Issues You’re Experiencing or Experienced


Be aware of your own body. When getting a body check package, health care providers may ask specific questions about your current conditions. If there are any changes you’ve experienced, write them down. Keep a note of your eating habits and any medications you’ve taken or currently taking. When your doctor asks you of health issues, answer honestly. 

Vaccinations and Screenings
